3JUVE in focus: scar revision

Scar formation is a complex interplay of different factors, and the degree and type of scarring can be influenced by several variables, including the size, depth and position of the wound, the blood supply to the region, the thickness and colour of the skin, and the person’s age. Further, there are numerous types of scars, the most common being atrophic, keloid and hypertrophic, as well as different types of acne scars. In some instances, it is possible to encounter combination scars, such as hypertrophied postoperative scars or keloid trauma wounds.

CO2 and Er:YAG lasers are commonly selected for the treatment of scars due to their ability to smooth scar texture and stimulate collagen production. While CO2 lasers emit a longer wavelength and can penetrate deeper into the skin, the high energy levels of CO2 lasers require a longer recovery time and can cause hyperpigmentation and other adverse effects in patients with higher Fitzpatrick skin types.

Erbium YAG lasers have a lower penetration depth and produce less thermal damage to surrounding tissue, resulting in reduced risk of side effects. The adjustable energy levels and precise wavelength of Er:YAG lasers make them suitable for treating a broader range of skin types. Erbium YAG laser treatments typically result in considerably less redness, swelling and post-treatment discomfort compared with CO2 laser treatments.

Lynton’s ResurFace® Er:YAG fractional laser (2940nm) technology, an integral part of the 3JUVE platform, has proven to be an invaluable addition to the aesthetic practitioner’s options for treating various types of scars – including challenging acne scars, surgical scars and traumatic scars.

We have seen remarkable scar revision results using the ResurFace® laser on the body, with significantly shorter healing times.’

A combination approach

‘It’s quite rare that a scar neatly fits in one category,’ says Dr Hills. ‘For example, if the scar is raised, it often has discolouration as well. If you have a red scar, you can employ various laser technologies to improve the scar itself, but to reduce the redness you need a system that works on vascular lesions, such as IPL.
