What is the role of human-derived exosomes in skin rejuvenation and regeneration?

Exosomes, both endogenic and exogenous, are being explored for their potential to repair, regenerate and rejuvenate skin tissue. They carry bioactive molecules that can influence various cellular processes essential for skin health. They are powerful molecules that up- and down-regulate critical processes of cellular repair, and also have a key role in reducing cellular senescence.

Exosomal therapy is emerging as a novel treatment option in regenerative aesthetics, the newest sub-category in the aesthetics world today. The secretome of human-derived stem cells, which includes exosomes, contains growth factors that have strong mitogenic activities, aiding in healthy tissue regeneration and skin rejuvenation. In short, a pure exosome will find damage and either repair it, or eliminate it, and will find healthy cells, and promote healthy growth
of more of those cells.

When it comes to exosomes, source is very important, but there are other important elements, too, such as how the exosomes are derived, stored and processed. Préime has gone to great lengths to ensure that their exosome is minimally processed, and alleviates the need altogether to go to cell culture and introduce foreign protein medium like many other brands do. This allows us to get better outcomes with less material, so we can truly target regenerative sites in skin.

Can you comment further on Préime’s sourcing and extraction methods and why this is important to the end product?

Préime’s exosome is derived from human umbilical cord blood plasma. Their patented extraction process derives the exosome directly from the blood, allowing for the purest harvest possible. This allows them to completely skip going to cell culture, as well as alleviate the introduction of foreign bovine serum, removing sources of contamination and additional particulate ending up in the final formula. This gives me the confidence that the product is pure, and I can be comfortable with what is going onto my patient’s skin. The end product is also reported to be highly stable due to the extraction process, which is important when it comes to working with proteins.

How do you use Préime’s exosome products in practice?

We use them in conjunction with our top aesthetic treatments and procedures, including both ablative and non-ablative lasers, as well as with microneedling. They are also given for daily-use homecare to maintain results at home.

What results have you been observing both as an adjunct to other cosmetic procedures and as a standalone treatment?

In a pilot study we recently did on Préime’s Pure exosome, we saw results in both groups – laser plus exosome, as well as exosome alone with no laser treatment. This is impressive because previous thinking maintained that you need some form of assisted delivery to get great results out of an exosome. However, in this case, due to the small size of exosomes, as well as the purity of Préime’s product, we are able to see regenerative benefits without being paired with an aesthetic treatment.

When combined with an ablative laser treatment, we identified the skin heals faster as compared to traditional standard emollient post care and the results are remarkable. Blinded evaluators noted improvement in wrinkle depth, hyperpigmentation and overall skin volume. Additionally, the majority of the treated patients said they were very satisfied with the outcome, and they would recommend it to a friend. We are now starting a more comprehensive study to further evaluate the efficacy based on the pilot trial insights.
