How has the Clear + Brilliant laser system helped to advance your aesthetic practice?
We were one of the first practices to acquire the original Clear + Brilliant laser in 2011 with the 1440nm handpiece. It revolutionised our practice by replacing all of our microdermabrasion treatments with superior non-ablative resurfacing of Clear + Brilliant.
Patients embraced the procedure and saw better results in the tone, texture and colour of their skin. The arrival of the 1927nm handpiece in 2012 gave us even more options with pigment.
Clear+ Brilliant has been as essential part of ongoing maintenance for my many patients who have had more aggressive resurfacing such a CO² and Fraxel Dual. We also find it useful as an introductory laser for resurfacing-new patients. Clear + Brilliant is my go-to for pigmentation associated with melasma and other pigmentary issues.
Many of my patients are simply delighted with the results achieved with Clear + Brilliant and don’t wish to proceed with more aggressive treatments.
This is a very delegable procedure with simple, clear-cut treatment protocols and defined energy and density settings, which are build in to three choices: Low, Medium and High. The Intelligent Optical Tracking makes the procedure easily reproducible and reliable.
Patient experience is not unpleasant, although we do generally use a topical anesthetic for the treatment. It fits in nicely in a busy office and augments more aggressive treatments.
Patient satisfaction levels have been consistently very high since we started offering Clear + Brilliant treatments 13 years ago.
Why did you transition to the new Clear + Brilliant Touch system?
Treatments are seamlessly performed with either or both 1440nm and 1927nm wavelengths without having to swap out the handpieces. Treatments are smoother, faster and the patient experience more streamlined; the user does not have to stop, change the handpiece and insert a new treatment card.
What feedback have you received from patients regarding their experiences with Clear + Brilliant Touch?
Patients truly love the Clear + Brilliant Touch. They find the treatments easy, convenient (little or no prep or downtime) and effective. Not uncommonly, patients will schedule every month or so, as they might a facial, but with more significant benefits to the health and wellness of the skin.
What has been the business impact since upgrading to the Clear + Brilliant Touch system?
New technology is always a boost to patient interest and adoption of a procedure. Knowing you have the latest and greatest devices inspires patient confidence.
Clear + Brilliant Touch’s dual treatments are faster and more cost effective and have a positive impact on our practice. One treatment (1 card used) allows treatment with either handpiece or both, if appropriate.

I have been a loyal Solta physician for more than 20 years, starting with the original Thermage, then Fraxel, followed by the original Clear + Brilliant.

What are the synergistic benefits of combining Clear + Brilliant Touch with other treatment modalities?
During my initial consultation, I emphasise the importance of optimising skin quality as the foundation of any anti-ageing treatment plan. Regardless of excellent bone structure or skillful use of fillers and neuromodulators if the skin is significantly sun-damaged, with pigmentation and deep wrinkles, the face will still appear aged.
I have used Clear + Brilliant Touch to effectively address various concerns, including photodamage, skin dullness, pigmentation, melasma and acne scarring. I also recommend the Clear + Brilliant Touch as a maintenance laser between more aggressive resurfacing treatments. My patients particularly enjoy scheduling their Clear + Brilliant Touch treatment 4-6 weeks before upcoming events.
What results have you observed when combining Clear + Brilliant Touch with Thermage FLX?
Thermage FLX combined with Clear + Brilliant Touch offers an ideal treatment for anti-ageing. I view Thermage FLX as a way to slow down the ageing process, or as they say say in Korea, ‘collagen banking’, by restoring elasticity to the skin and effectively improving skin laxity. Following up with Clear + Brilliant Touch addresses skin texture and pigmentation, resulting in tighter, brighter skin. My patients love the refreshed look and often find they no longer need makeup daily to feel ‘natural looking’.
How does Clear + Brilliant Touch compare to Fraxel in terms of treatment outcomes and patient satisfaction?
Fraxel and Clear + Brilliant are two distinct lasers but equally popular among my patients. I consider Fraxel as a powerful tool for deeply resurfacing the skin, while Clear + Brilliant serves as a maintenance treatment in between sessions.
Patients appreciate how easily Clear + Brilliant fits into their busy lifestyles, given its minimal downtime. In contrast, Fraxel requires a greater commitment to post-treatment care and recovery.
Why is it important to you to offer your patients a multi-modality approach?
Ageing is multi-factorial. My approach to cosmetic treatments considers the whole person, factoring in their anatomy, lifestyle, daily stressors and societal expectations. Innovative treatments for diverse skin types

What are the key aesthetic device considerations for skin of colour?
When it comes to lasers and other energy-based devices in the aesthetics market, it is important to keep physics in mind. Most of these devices work by depositing heat
or excited light energy into a target (chromophore) that corresponds with a light wavelength and colour. The device works best when all the energy is confined to the chromophore only. Things become risky if the heat and energy are not contained and cause collateral damage to surrounding structures. In the case of a patient with pigment-rich skin, that could mean damaging melanocytes or melanin to the point where the patient develops hyperpigmentation or hypopigmentation.
Some patients of colour are also more prone to developing exaggerated scars, or keloids. Improper use of energy-based devices may also put a patient at risk for development of tough-to- treat keloids.
What considerations should be taken when developing treatment protocols for darker skin types?
The first is to understand what the intended chromophore will be and what parameters will increase the risk of collateral damage and adverse effects. This requires proper training in the use of the device you are seeking to deploy.
Next, you need to fully understand what goal the patient is trying to achieve, and if you have a device that can achieve that goal safely.
One size does not fit all. Take the time to customise a plan for the patient in front of you. Once a plan has been established, determine what device setting scan maximise your chances of a safe and successful outcome. That means selecting the right energy level, pulse duration, time interval between pulses and, if applicable, a method of cooling tissue before or after pulses.
How do you mitigate risks in patients with darker skin while achieving optimal efficacy and results?
The first step is to select a device that is either going to ignore the patient’s melanin as a direct target or is at least capable of selectively treating the target tissue while simultaneously protecting the patient’s pigment. This can mean targeting a chromophore like water while setting the device parameters to limit spread of heat away from the dermis. It can also mean using pre- or post-cooling measures to dissipate heat that may migrate towards the epidermis. When melanin itself is the target, care must be taken to select the right wavelength and energy settings to gently eliminate the unwanted pigmentation while sparing the patient’s natural skin tone. This requires care and proper training.
Can you comment on the application and outcomes of energy-based treatments on Asian skin?
Many of these patients have Fitzpatrick types III-VI skin and therefore careful consideration must be taken in selecting and executing their laser procedures. Wavelengths and pulse durations that very carefully treat melanin must be used judiciously. Measures to promote post-treatment cooling must also be considered and deployed when protecting melanocytes is necessary.
The results patients enjoy when these treatments are performed correctly can be stunning. Some of the most dramatic outcomes we have seen in my practice have been in Asian men and women following laser procedures. Their success has been a springboard for growing my practice as they are often eager to spread the news about their great results to other prospective patients.
Can you describe the mechanism of action of the new Clear + Brilliant Touch?
The Clear + Brilliant Touch is a dual wavelength integrated laser system. Both the 1440nm wavelength and the 1927nm Permea wavelength are non-ablative fractionated resurfacing treatments that use water as their chromophore. The dual wavelength nature of the device allows for seamless treatment of pigmentary disorders and structural abnormalities of the dermis in one integrated treatment.
The device easily toggles between the 1440nm and 1927nm laser wavelengths, and each laser’s number of passes and energy output can be customised independently. The two lasers operate at different depths within the target tissue, allowing for two distinct effects. The longer wavelength promotes skin brightening and the shorter wavelength enhances dermal collagen remodelling, promoting smoother texture.
I love this device and what is has offered to my practice: an increase in new patients, better engagement of my existing patients, and robust bottom-line growth.
How does Clear + Brilliant Touch enhance your current treatment offering diverse skin types?
My practice already treats patients with all Fitzpatrick skin types, so I am accustomed to offering different treatments that can be tailored to the needs of my diverse patient base. The Clear + Brilliant Touch is one of the few devices in my armamentarium that is appropriate for treating all Fitzpatrick skin types. This has made it easy to attract new patients and convert existing patients to loyal devotees of this device. The results it delivers are real and predictable; patients who receive regular treatments often tell me their friends and acquaintances compliment them on their clear skin!

What an aesthetic practitioner really needs to do is make sure they have the right amount of knowledge and training to safely administer energy- based treatments in different skin phototypes.
