The benefits of radiofrequency energy in skin rejuvenation have been widely documented. A thermal response is created within the skin during treatment, which is caused by the resistance of the skin to RF energy. Due to this, tissue coagulation and skin tightening occurs.

Infini vs monopolar and bipolar RF

Both monopolar and bipolar RF have been reported for the treatment of wrinkles – but the
results are often inconsistent. Bipolar RF involves current passing through the tissue between two electrodes, and monopolar is RF energy projected into the skin from a single source.

Both methods result in sub-optimal tissue warming – somewhere below 65 degrees Celsius. Because of this, they have been shown to lead to limited and inconsistent clinical outcomes.

In comparison, Infini delivers RF energy via insulated gold-coated microneedles to precise depths within the skin. This affords deep and controlled coagulation, giving rise to optimised neocollagenesis.

Treatment practicalities

The insulated gold-coated microneedles mean treatment is suitable for all skin types in any season. The needles help protect the epidermis from thermal damage, and can target tissue more than 3.5mm deep.

The needle depth can be adjusted for a multi-layered approach. This can help in treatment customisation for individual patients, and achieves 3D results in skin rejuvenation.

The Infini system can be used on eyelids, and other facial zones – such as periocular, nose, maxillary, temple, forehead and perioral skin. These zones can be targeted with needles between 0.5 and 3.5mm.

Infini’s Comfort Flow Technology administers sequential needle insertion to minimise patient discomfort. Typically, needle depth is set for the first pass across the whole face and neck. Second and subsequent passes can be performed over areas of concern with lower needle depths and various parameters. As the needle depth is reduced, the level and exposure time should also be lowered to avoid secondary thermal damage to the epidermis.

Results with Infini

Infini is touted to induce 3D shrinkage of the target tissue, followed by volumetric changes due to dermal neocollagenesis and elastogenesis.

To test the efficacy and safety of the system, a multicentre study was conducted across various geographic locations and different skin types. It encompassed 499 subjects – 419 females, 80 males and five countries. The mean age was 52.6 years, and the Fitzpatrick skin types ranged between I and V.

All centres reported very good to excellent results, ranging from mild to excellent improvement in even severe wrinkles. Some improvement was noted in every subject, in all centres, and results in the neck were judged better than any
other modality.

No persistent side effects were seen after Infini treatment, with no post-inflammatory pigmentation reported in skin types III-V.
