A new eye-tracking study has offered fresh insight into the role of the nose in facial attractiveness, suggesting the most aesthetically pleasing noses are those that do not draw attention to themselves.
Published in Plastic and Reconstructive Surgery, the study examined how nasal shape, perceived attractiveness and visual attention interact, using a combination of facial imaging, attractiveness ratings, three-dimensional anthropometric measurements and eye-tracking technology.
The study included 34 models with no history of facial surgery or congenital facial deformity, and 31 observers who assessed facial and nasal attractiveness. Observers viewed standardised frontal and lateral facial images while an eye-tracker recorded how long they looked at different regions of the face, including the eyes, nose and mouth.
The findings support a concept familiar to many aesthetic practitioners: facial harmony is often less about one feature standing out, and more about how well each feature integrates with the rest of the face.
When the nose draws the eye
According to the study, noses perceived as unattractive received significantly more visual attention than noses rated as attractive. In frontal view, observers spent longer looking at noses perceived as unattractive, with greater fixation duration and more revisit counts.
By contrast, faces with noses perceived as attractive drew more visual attention to the eyes. The researchers suggested this may indicate that an aesthetically pleasing nose supports overall facial harmony by allowing the gaze to settle on more socially expressive facial features, rather than pulling attention toward the centre of the face.
The study also found that noses perceived as unattractive were associated with increased attention to the mouth, which the authors suggested may reflect a compensatory gaze pattern when a facial feature is perceived as disharmonious.
Importance of facial context
Interestingly, the research challenges the idea that nasal aesthetics can be fully understood through isolated measurements or traditional aesthetic canons. While nasal tip deviation and longer upper lips were associated with lower attractiveness ratings, nasal width and several traditional neoclassical proportions had limited impact in this cohort.
Nasal attractiveness ratings were also lower when observers assessed cropped, nose-only images compared with when the nose was viewed within the full face. This reinforces the importance of evaluating the nose in facial context rather than as an isolated structure.
The authors noted that nasal tip deviation was significant when the nose was assessed in isolation, but this association disappeared when the nose was viewed as part of the full face. For aesthetic surgeons, this finding may be particularly relevant during consultation and treatment planning, where patients may focus heavily on a specific nasal concern that does not necessarily carry the same visual weight in the broader facial setting.
Implications for rhinoplasty planning
The study adds to growing interest in eye-tracking as a tool for understanding facial aesthetics. Previous research has explored how visual attention changes before and after rhinoplasty, particularly in patients with crooked or uncorrected noses. This study differed by assessing natural nasal variation in a nonsurgical, demographically diverse group.
For rhinoplasty consultations, the findings may help support discussions around realistic expectations and facial balance. Rather than framing success as creating a nose that is visually prominent or idealised in isolation, the study points to the value of a nose that blends with the surrounding facial features and supports attention to the eyes. This may be particularly relevant during consultation and treatment planning, where patients may focus heavily on a specific nasal concern that does not necessarily carry the same visual weight in the broader facial setting.
The authors concluded that noses perceived as attractive enhance facial harmony by drawing less visual attention to themselves. They also emphasised the need for holistic facial assessment in rhinoplasty planning, as well as further research using dynamic facial stimuli, larger samples and more diverse observer populations.
